LifeScape offers Family Support 360 services to people with intellectual/developmental disabilities and their families in Sioux Falls and the surrounding communities. We help both children and adults—and their families—in self-directing the services they need to live as independently as possible in the community with their family or in their own home. Our Family Support Coordinators are trained in person-centered planning and focus on personal outcomes for the people they support. Combined, our coordinators have over 70 years of service to people with disabilities.
There are services and supports available to people with developmental disabilities that you may not be aware of. A Family Support Coordinator could help you to apply for and maintain funding, develop an Individualized Service Plan (ISP) to track and assess needs and services, identify and prioritize needs and provide support through the Individualized Education Plan (IEP) process. We can help connect you to financial assistance for costs related to having a disability, to supports through community organizations, and to respite or companion care.
